Health and wellbeing

It’s important to take care of your physical and mental health, to ensure you stay happy and healthy and we will support you in doing so.

Leisure offer

In partnership with Everyone Active, Slough Children First offers free gym membership to you up to the age of 25.

Your Social Worker or PA will complete the online secure referral form and following this, you’ll receive an email detailing the next steps.

Health Passport

A Health Passport is a small booklet given to all young people before they leave care.

Your Health Passport gives you a summary of your health history and immunisations and is for you to keep and update at any time.

It can be translated for you if English is not your first spoken language and we can also deliver it to you electronically or by post.

General health advice

The NHS website will give you information and advice on a range of health and wellbeing topics, including where to find local health services. You can also download the app too.

SCF’s Children Looked After team has also developed the following website that contains links

Feeling unwell?

Feeling unwell but don’t need to call 999 for emergency help?

You can make an appointment
with your GP or ask your local pharmacist for advice for many common minor illnesses such as
diarrhoea, minor infections, headache, travel advice or sore throats.

You can also call 111 – the NHS number to dial for non-emergency medical concerns.

Young parents

Your YPA will help to support you
in planning for the arrival of your child and accessing post birth support.

Your YPA will be able to link in with pre and ante natal care services, housing and benefit agencies to
ensure that you are provided with the care and support you need. Some of the services available to

Free prescription card

The NHS is now able to offer free prescription cards for (eligible) care leavers.

NHS Frimley are committed to supporting care leavers with their health and wellbeing and are providing financial support to pay for prescriptions.

Speak to your Personal Advisor or contact the Leaving Care Team, and they will be able to support you in the process. You can also contact the Designated Nurse at frimleyicb.safeguarding@nhs.net
