My Review…My Views For children aged 8 to 11

My carer has spoken to me about my My Review My Views form and I am happy to use it to share my views and feelings?
Name
If someone is helping you to complete your form, please can you tell us their name:
Email address
When you have finished your form we will make sure you are emailed a copy. This can be your carers email or your own email if you are aged 13 and over.

Your review

Is this your first review?
Your social worker and IRO will be at your review. Has your social worker asked you who else you would like to be there, for example, your carer or teacher?
Has your social worker asked you where you would like your review to be held, for example, at school or at home?
All children who are cared for by a foster carer can have someone called an ADVOCATE. This is someone who can help you to share how you are feeling. They will listen to you and make sure that all of the adults that are caring for you hear what you have to say.

Where you live

Do you know understand why you are looked after by your foster carer?
Tell us about your activities and hobbies. You can also tell us about any activities or hobbies that you would like to do.
Tell us about anything you need to support you with your religion, language or culture. For example, you may need special food or clothes.
