Salary: £30,139 – £34,451 (including local weighting of £965)

You will work with families at various times including early mornings and evenings, delivery services where they are required such as the family home and community settings.
You will act as a dedicated lead professional at all stages of support from referral through to exit strategy.
Main Accountabilities
- To conduct comprehensive whole family assessments as required and produce appropriate written reports detailing family assessment and ensuring that these are recorded on the EHM system.
- Deliver targeted interventions to meet child and family needs, reducing risk and leading to improved sustainable outcomes.
- To act as dedicated lead professional at all stages of support from referral through to exit strategy, using support and challenge measures to ensure that parents and children engage and comply with the family’s action plan.
- Coordinate support from external agencies and lead the Team Around the Family (TAF) approach.
- To work with families at various times including early morning and evenings, delivering services where they are required such as the family home and community settings.
- To maintain effective records of family interventions and use the outcomes-based family start assessment tool to evidence the achievement of outcomes and enable evaluation recording these on the EHM system.
- To ensure all interventions are conducted within an anti-discriminatory framework, and take into account issues such as race, gender, sexuality, disability etc, and deliver appropriate services to the diverse communities of Slough.
- The post holder will be expected to take on additional duties and changes to the job description as required by your line manager commensurate with the post.

Education, Training & Experience
- Educated to ‘A’ level/NVQ Level 3 or equivalent with practical knowledge of applied business practices including, where relevant, basic awareness of relevant legislation. May require additional technical or specialist training. (Essential)
- Demonstrable commitment to continuous professional development. (Essential)
- Experience acquired through a combination of training and working in a relevant area; demonstrating self development and an enthusiasm to learn. (Essential)
- Experience of working successfully in teams, across teams and partnerships. (Essential)
- Proven track record of embracing digital approaches and new ways of working. (Essential)
- Membership of a relevant professional body. (Desirable)
General & Specific Knowledge
- Good level of digital literacy, including proficient use of office software suites and flexible ways of working (e.g. video conferencing, remote working). (Essential)
- Interpret and analyse performance and finance information to feed into practice improvements and contribute to business planning. (Essential)
- Ability to plan and organise work to produce a high level of output to quality standards. (Essential)
- Develop productive and collaborative working relationships with colleagues and stakeholders. (Essential)
